TRANSITIONS IS A PROGRAM COMPONENT FOR CHILDREN AND FAMILIES IMPACTED BY DIVORCE, SEPARATION, AND CUSTODY. SERVICES INCLUDE ASSESSMENT, CONFLICT RESOLUTION, COMMUNICATION COUNSELING, SUPERVISED VISITATION, THERAPEUTIC REUNIFICATION, AND CO-PARENTING EDUCATION.
GENERATIONS/GROWING TOGETHER - GENERATIONS IS A PROGRAM COMPONENT DESIGNED TO MEET THE SPECIAL NEEDS OF GRANDPARENTS AND OTHER RELATIVES RAISING CHILDREN. CURRENT SERVICES INCLUDE SUPPORT AND FORMAL KINSHIP CARE. SERVICES ALSO INCLUDE A CLINICAL COMPONENT. GROWING TOGETHER IS A PROGRAM COMPONENT FOR PARENTING ADOLESCENTS STRIVING TO MAINTAIN CUSTODY OF THEIR INFANT WHILE IN FOSTER CARE. THE YOUTH AND HER INFANT WILL BE PROVIDED THE FOLLOWING SERVICES: THERAPEUTUC FOSTER HOMES, RESPITE CARE AND SPECIAL EVENTS CHILD CARE, GROUP, FAMILY, AND INDIVIDUAL THERAPIES, INCENTIVES FOR ACTIVE PROGRAM PARTICIPATION, AND RECREATIONAL AND AGE APPROPRIATE ACTIVITIES.
FOREVER FAMILY IS A PROGRAM FOR ADOPTION AND A HOST OF PERMANENCY SERVICES DESIGNED TO SUPPORT CHILDREN AND PARENTS AS THEY STRIVE TO EXPAND THEIR FAMILIES. THIS PROGRAM IS LICENSED BY THE PENNSYLVANIA DEPARTMENT OF HUMAN SERVICES AND IS AN AFFILIATE OF THE STATEWIDE ADOPTION NETWORK.
