SERVICE COORDINATION SERVICES: THIS SERVICE ASSISTS PARTICIPANTS IN GAINING ACCESS TO NEEDED HOME AND COMMUNITY BASED SERVICES, AS WELL AS MEDICAL, SOCIAL, EDUCATIONAL AND OTHER SERVICES REGARDLESS OF THE FUNDING SOURCE. SERVICE COORDINATION IS WORKING WITH AND AT THE DIRECTION OF THE PARTICIPANT WHENEVER POSSIBLE TO IDENTIFY, COORDINATE AND KEEP PEOPLE OUT OF INSTITUTIONS AND ALLOW THEM TO LIVE IN THEIR COMMUNITIES. OVER 4,200 PARTICIPANTS CURRENTLY RECEIVE SERVICES THROUGH THIS PROGRAM, IN 52 COUNTIES IN PENNSYLVANIA. THIS PROGRAM WAS AWARDED A 3-YEAR ACCREDITATION FROM THE NATIONAL COMMITTEE FOR QUALITY ASSURANCE (NCQA), SHOWING THAT THIS PROGRAM MEETS EVIDENCE-BASED BEST PRACTICES TO ENSURE EFFICIENT AND COST-EFFECTIVE CASE MANAGEMENT PROCESSES AND SERVICE DELIVERY.
