ADULT DAY CARE:THE ADULT DAY ACTIVITIES PROGRAM (ADAP) AT RIVERDALE SENIOR SERVICES IS A SOCIAL-MODEL DAY PROGRAM FOR PEOPLE WHO HAVE MEMORY LOSS OR ARE PHYSICALLY FRAIL. ADAP OFFERS STRUCTURED ACTIVITIES IN A SOCIAL ENVIRONMENT, AS WELL AS PERSONAL CARE ASSISTANCE AND SUPERVISION. BY OFFERING A SAFE AND SUPPORTIVE ENVIRONMENT, PEOPLE WITH MEMORY LOSS OR PHYSICAL FRAILTIES CAN MAINTAIN THEIR INDEPENDENCE IN THE COMMUNITY, HEIGHTEN THEIR WELL-BEING AND SELF-ESTEEM, AND REDUCE ISOLATION AND BOREDOM.
PROGRAMS & ACTIVITIES FOR OLDER ADULTS:THE ORGANIZATION PROVIDES SERVICES BY IDENTIFYING THE NEEDS OF ELDERLY RESIDENTS OF THE BRONX. SUCH SERVICES INCLUDE PROVIDING THE ELDERLY WITH MEALS, CASE ASSISTANCE, HEALTH MANAGEMENT, ART, PHYSICAL HEALTH & EXERCISE, NUTRITION, AND TECHNOLOGY CLASSES.
TRANSPORTATION:THE TRI-CENTER TRANSPORTATION PROGRAM PROVIDES SPECIALIZED TRANSPORTATION FOR THE FRAIL ELDERLY. EACH YEAR, NUMEROUS RIVERDALE SENIOR SERVICES MEMBERS AS WELL AS JASA VAN CORTLANDT VILLAGE SENIOR CENTER PASSENGERS, INCLUDING THOSE WITH WHEELCHAIRS, ARE SERVICED FIVE DAYS A WEEK WITH TRANSPORTATION. THE HANDICAPPED ACCESSIBLE BUSES TAKE OLDER ADULTS TO AND FROM THEIR HOME FOR A HEALTHY LUNCH AND PLANNED ACTIVITIES AT RIVERDALE SENIOR SERVICES. THE PROGRAM ALSO CONNECTS CLIENTS TO A FULL RANGE OF SERVICES, SUCH AS CASE ASSISTANCE, NUTRITION COUNSELING AND MORE. IN ADDITION TO ITS ESTABLISHED ROUTES, THE TRI-CENTER PROGRAM PROVIDE MEDICAL RIDES FOR OLDER ADULTS WHO NEED TRANSPORTATION FOR LOCAL DOCTOR VISITS.
TO PROVIDE COORDINATION OF RESOURCES PROVIDED THROUGH BON SECOURS HEALTH SYSTEM MISSION FUND GRANTS TO IMPROVE HEALTH, SOCIAL DETERMINANTS, AND OVERALL CONDITIONS FOR VULNERABLE COMMUNITIES.
