WINNING WHEELS CONSISTS OF CARE CENTER, AN 88-BED SKILLED NURSING FACILITY AND S.T.R.I.V.E. (SOCIALIZATION, TRAINING, REHABILITATION, INDEPENDENCE, VOCATION, EDUCATION), A 16-BED INTERMEDIATE CARE FACILITY, SERVING THE DEVELOPMENTALLY DISABLED SEGMENT OF THE CLIENT POPULATION.
AL'S PLACE OPERATES AND MANAGES PINNACLE PLACE AND FRONTIER HOLLOW, WHICH ARE APARTMENTS OCCUPIED BY ELIGIBLE LOW-INCOME INDIVIDUALS. PINNACLE PLACE AND FRONTIER HOLLOW ARE LOCATED IN SAVANNA, AND PROPHETSTOWN, ILLINOIS. LOW-INCOME INDIVIDUALS RESIDING AT PINNACLE PLACE AND FRONTIER HOLLOW HAVE ACCESS TO HOMEMAKER SERVICES, MEALS, AND OTHER ASSISTED CARE SERVICES.
