THE MISSION OF THE ORGANIZATION IS TO PROVIDE ESSENTIAL AND INNOVATIVE RESOURCES FOR INDIVIDUALS, FAMILIES AND COMMUNITIES TO PREPARE FOR AND SUPPORT GRACEFUL AGING. THIS IS ACCOMPLISHED PRIMARILY THROUGH PROVIDING A VARIETY OF HOME AND COMMUNITY BASED SERVICES FOR THE ELDERLY, THE HANDICAPPED, AND THEIR CAREGIVERS.

URBAN JACKSONVILLE, INC. IS AN APPROVED ACHC AND MEDICARE CERTIFIED HOME HEALTH AGENCY AND PROVIDES HOME HEALTH SERVICES PRIMARILY TO ELDERLY AND HANDICAPPED PERSONS. SERVICES INCLUDE SKILLED NURSING; PHYSICAL, OCCUPATIONAL AND SPEECH THERAPY; MEDICAL SOCIAL WORK SERVICES; AND HOME HEALTH AIDE SERVICES. DURING THE YEAR ENDED 09/30/2021, 811 CLIENTS WERE SERVED.
THE MEALS ON WHEELS (MOW) PROGRAM PROVIDES QUALIFIED ELDERLY OR DISABLED PEOPLEWITH HOT, NUTRITIOUS MEALS DELIVERED TO THEIR HOMES EACH DAY. MOW ALSO DELIVERS FROZEN AND SHELF-STABLE MEALS WHEN SUCH MEALS ARE INTENDED TO BE EATEN ON DAYS SUBSEQUENT TO THEIR DELIVERY. 228,156 MEALS WERE DELIVERED DURING THE YEAR ENDED 9/30/2021.
RESPITE SERVICES PROVIDE IN-HOME ASSISTANCE AND RELIEF TO CAREGIVERS OF FRAIL, HOMEBOUND ELDERS AGED SIXTY AND OVER. THE INTENT OF RESPITE IS TO PROVIDE FAMILY CAREGIVERS WITH A MUCH-NEEDED BREAK IN ORDER TO ALLOW THE FAMILY UNIT TO CONTINUE CARING FOR A HOMEBOUND ELDER WITHOUT BECOMING ILL THEMSELVES. THE ORGANIZATION PROVIDED 76,664 HOURS OF RESPITE SERVICES DURING THE YEAR ENDED 9/30/2021.
OTHER PROGRAM SERVICES INCLUDE A NUMBER OF PROGRAMS THAT ARE DESIGNED TO MAINTAIN THE ELDERLY AND DISABLED IN THEIR OWN HOMES FOR AS LONG AS POSSIBLE, AVOIDING PREMATURE INSTITUTIONALIZATION. THIS INCLUDES VARIOUS PROGRAM SERVICES INCLUDING HOMEMAKING, PERSONAL CARE, MEDICAL TRANSPORTATION, COUNSELING, COMPANIONSHIP, ADULT DAY CARE, RESPITE, SPECIALIZED MEDICAL EQUIPMENT, EMERGENCY ALERT RESPONSE, AND CAREGIVER TRAINING AND SUPPORT.
