SERVED 1,045 PATIENTS AND MADE 22,691 HOME VISITS. CARE PROVIDED TO INDIVIDUALS OF ALL AGES THROUGH ALL STAGES OF LIFE AND HAVE AN 87% PATIENT SATISFACTION FOR HOME HEALTH AND 93% PATIENT SATISFACTION FOR HOSPICE. THE PATIENT SATISFACTION RATINGS COVER THE PERIOD JANUARY 1, 2021 THROUGH DECEMBER 31, 2021.
