THE ORGANIZATION PROVIDES AN OUTREACH PROGRAM TO ASSIST OLDER ADULTS, AGES 60 AND OLDER, TO REMAIN IN THEIR HOMES, IN THE NEIGHBORHOODS OF GERMANTOWN, MT. AIRY, CHESTNUT HILL, WEST OAK LANE, AND A SECTION OF EAST FALLS. THE ORGANIZATION OFFERS HOME VISITS TO ASSESS INDIVIDUAL NEEDS AND PROVIDE HEALTH EDUCATION AND SOCIAL SERVICES TO PROVIDE INDIVIDUALIZED CARE MANAGEMENT AND COUNSELING, HEALTH SCREENING, EDUCATIONAL SEMINARS AND OVERALL CARE SUPPORT WITH OTHER PROVIDERS AND ORGANIZATIONS IN THE NORTHWEST PHILADELPHIA AREA.
