HEALTH PROMOTION ENCOURAGES OLDER ADULTS TO SET AND ACHIEVE THEIR WELLNESS GOALS THROUGH HEALTH EDUCATION AND WELLNESS WORKSHOPS, INDIVIDUAL CONSULTATION, AND HEALTH SCREENINGS. SOCIAL SERVICES PROVIDE INDIVIDUALIZED CASE MANAGEMENT AND COUNSELING AND ACCESS TO COMMUNITY RESOURCES. IN ADDITION, THE ORGANIZATION PROVIDES SUPPORTIVE SERVICES FOR THE RESIDENTS OF THE MAPLE VILLAGE APARTMENTS, WHICH IS WHERE THE ORGANIZATIONS OFFICE IS LOCATED. CLIENT ASSISTANCE FUND PROVIDES SMALL GRANTS TO OLDER ADULTS IN NEED.
