FACT (FAMILIES AND COMMUNITY TOGETHER) IS PART OF THE NJ CHILDREN'S SYSTEM OF CARE AND IS THE DESIGNATED CARE MANAGEMENT ORGANIZATION FOR UNION COUNTY. WE EXIST TO HELP THE YOUTH OF UNION COUNTY TO SUCCEED AT HOME, IN SCHOOL AND IN THEIR COMMUNITY. WE WORK WITH YOUNG PEOPLE (FROM AGES 5 UP UNTIL THEIR 21ST BIRTHDAY) AND THEIR FAMILIES WHO HAVE EMOTIONAL, BEHAVIORAL, DEVELOPMENTAL, MENTAL HEALTH, AND SUBSTANCE ABUSE NEEDS. WE PRACTICE A WRAP AROUND APPROACH, WHICH IS A FAMILY ORIENTED AND COMMUNITY-BASED MODEL OF CARE THAT ADDRESSES BARRIERS TO WELL-BEING, MAXIMIZES THE YOUTHS AND FAMILIES' STRENGTHS, AND HELPS THE FAMILY IDENTIFY AND BUILD A TEAM OF SUPPORTS.
