NURSING HOME TRANSITION AND DIVERSION - TO PROVIDE SUPPORTS AND SERVICES TO ASSIST INDIVIDUALS WITH DISABILITIES AND SENIORS TOWARD SUCCESSFUL INCLUSION IN THE COMMUNITY. PARTICIPANTS MAY COME FROM A NURSING FACILITY OR OTHER INSTITUTION (TRANSITION), OR CHOOSE TO PARTICIPATE TO PREVENT INSTITUTIONALIZATION (DIVERSION).
