HPCFM IS DEDICATED TO ADVANCING AND PROMOTING EXCELLENCE IN END-OF-LIFE CARE BY:- ADVOCATING FOR MEMBERS, PATIENTS, FAMILIES AND THE END OF LIFE CAREGIVING COMMUNIT Y BY PROMOTING PUBLIC POLICY IN THE LEGISLATURE AND WITH REGULATORY AGENCIES THAT SUPPORT IMPROVEMENTS IN END-OF-LIFE CARE- SPONSORING EDUCATIONAL PROGRAMS THAT BUILD KNOWLEDGE AND SKILLS OF HOSPICE AND PALLIATIVE CARE PROFESSIONALS AND VOLUNTEERS- ENHANCING AWARENESS OF END OF LIFE CARE BY PROVIDING INFORMATION AND REFERRALS TO THE PUBLIC AS WELL AS HEALTH AND HUMAN SERVICE PROFESSIONALSFOSTERING INNOVATION TO IMPROVE END-OF-LIFE CARE THROUGH COLLABORATION, INFORMATION AND NETWORKINGLAST YEAR, HOSPICE AND PALLIATIVE CARE MEMBER PROGRAMS PROVIDED CARE TO MORE THAN 21,000 TERMINALLY ILL PATIENTS AND THEIR FAMILIES.
