INPATIENT: PATIENTS WHO ARE CRITICALLY ILL AND WHOSE CONDITIONS ARE RAPIDLY CHANGING REQUIRE CONSTANT ATTENTION AND ASSESSMENT. AT CONNECTICUT HOSPICE, A PATIENT'S CARE PLAN IS INDIVIDUALLY DESIGNED AND BASED ON CLINICAL ASSESSMENT AND CAREFUL DOCUMENTATION OF SYMPTOMS. OUR COMPREHENSIVE APPROACH ENABLES PATIENTS AND FAMILIES TO ATTAIN OPTIMUM QUALITY OF LIFE WITH FREQUENT PHYSICIAN AND CLINICAL PHARMACY ROUNDS EACH DAY ALONG WITH SUPERB NURSING CARE.
HOME CARE: SINCE 1974, CONNECTICUT HOSPICE HOME CARE NURSES HAVE PROVIDED SERVICES TO OVER 70 CITIES AND TOWNS IN CONNECTICUT. ON AN AVERAGE DAY, OVER 200 PATIENTS RECEIVE A MIX OF NURSING, SOCIAL WORK, MEDICAL, AND PASTORAL CARE AT HOME. OUR HOME CARE PROGRAM IS DIVIDED INTO STAND BY ME, A NON-HOSPICE PROGRAM FOR PEOPLE IN NEED OF SKILLED NURSING SERVICES WHO ARE STILL SEEKING CONVENTIONAL MEDICAL CARE SUCH AS CHEMO- AND/OR IMMUNOTHERAPY, AND HOSPICE HOME CARE, FOR THOSE ELECTING TO STOP CONVENTIONAL CARE AND SEEK COMFORT ORIENTED MEASURES ONLY.
