HOME HEALTH: THE HOME HEALTH PROGRAMS PROVIDES SKILLED NURSES, PHYSICAL THERAPISTS, OCCUPATIONAL THERAPISTS, SPEECH THERAPISTS, MEDICAL SOCIAL WORKERS, NUTRITIONISTS AND HOME HEALTH AIDES TO HOMEBOUND CLIENTS IN THEIR RESIDENCE. VNA STAFF ARE SPECIALIZED IN WOUND CARE, CERTIFIED DIABETIC EDUCATION, INFUSION THERAPY, CONGESTIVE HEART FAILURE, AND CASE MANAGEMENT. TREATMENT PLANS ARE COORDINATED WITH THE PHYSICIAN, THE PATIENT AND THE CAREGIVER TO HELP PATIENTS ACHIEVE THEIR OPTIMAL LEVEL OF FUNCTIONING AND INDEPENDENCE. THE PROGRAM PROVIDES HOME ASSESSMENTS, PATIENT EDUCATION AND SKILLED NURSING TREATMENTS AS ORDERED BY PHYSICIANS. A TELEHEALTH PROGRAM IS PROVIDED TO REMOTELY MONITOR VITAL SIGNS, WEIGHT AND KNOWLEDGE OF THE DISEASE PROCESSES FOR PATIENTS REQUIRING ADDITIONAL SUPPORT ON A DAILY BASIS.IN FISCAL YEAR 2021, THERE WERE 904 HOME CARE ADMISSIONS AND 13,381 HOME CARE VISITS WERE PERFORMED.
HOSPICE: THE HOSPICE PROGRAM WAS ESTABLISHED IN 1994 TO PROVIDE AN INTERDISCIPLINARY TEAM OF NURSES, THERAPISTS, MEDICAL SOCIAL WORKERS, CHAPLAINS, HOME HEALTH AIDES, NUTRITIONISTS AND VOLUNTEERS WORKING IN COLLABORATION WITH THE PHYSICIAN AND MEDICAL DIRECTOR TO CARE FOR PATIENTS FACING THE FINAL STAGES OF AN INCURABLE ILLNESS, GIVING THE SUPPORT NEEDED TO REMAIN IN THEIR HOMES OR RESIDENCES. HOSPICE TEAM MEMBERS PROVIDE DIRECT SERVICES TO ENSURE PAIN AND SYMPTOMS ARE MANAGED EFFECTIVELY AND TO EDUCATE PATIENTS AND CAREGIVERS ABOUT THE PROGRESSION OF ILLNESS. HOSPICE STAFF ALSO OFFERS SPIRITUAL AND EMOTIONAL SUPPORT TO BOTH PATIENTS AND CAREGIVERS. MEDICATIONS, EQUIPMENT, SUPPLIES, CONSULTATIONS, RESPITE CARE AND INPATIENT CARE ARE PROVIDED WHEN NEEDED.BEREAVEMENT SERVICES, MEMORIAL AND SUPPORT GROUPS ARE AVAILABLE FOR FAMILIES AND CAREGIVERS GOING THROUGH THE GRIEVING PROCESS. THERE WERE 120 HOSPICE ADMISSIONS AND 8,147 HOSPICE DAYS IN FISCAL YEAR 2021.
THE CASE MANAGEMENT UNIT WAS ESTABLISHED IN 1987 UNDER A CONTRACT WITH THE NEW JERSEY DIVISION OF MEDICAL ASSISTANCE TO PROVIDE CASE MANAGEMENT SERVICES TO CLIENTS ENROLLED IN THE FOLLOWING MEDICAID WAIVER PROGRAMS: GLOBAL OPTIONS, HIV HOMECARE PROGRAM, COMMUNITY RESOURCES FOR PEOPLE WITH DISABILITIES, AND AID COMMUNITY ALTERNATIVES. SERVICES INCLUDED CARE MANAGEMENT, RESPITE, HOMEMAKER, ADULT DAY CARE, NON-EMERGENCY MEDICAL TRANSPORTATION, EMERGENCY RESPONSE SYSTEMS TO CLIENTS WHO ARE AT RISK FOR NURSING HOME PLACEMENT TO REMAIN AT HOME WITH ADDITIONAL SUPPORT. THE CASE MANAGEMENT PROGRAMS THAT WERE PART OF THE NEW JERSEY MEDICAL ASSISTANCE OFFICE WERE EITHER DISCONTINUED OR TRANSFERRED FROM THE MEDICAID OFFICES TO THE MANAGED MEDICAID PAYERS.
