HEALTH CURRENT IS THE STATEWIDE HEALTH INFORMATION EXCHANGE (HIE) IN ARIZONA THAT HELPS PARTNERS TRANSFORM CARE BY BRINGING TOGETHER COMMUNITIES AND INFORMATION. THE HIE CONNECTS THE ELECTRONIC HEALTH RECORD (EHR) SYSTEMS OF HEALTHCARE PROVIDERS, ALLOWING THE SECURE SHARING OF PATIENT INFORMATION TO BETTER COORDINATE CARE. SINCE 2007, WE HAVE WORKED TO BECOME ARIZONA'S PRIMARY RESOURCE FOR HEALTH INFORMATION TECHNOLOGY (HEALTH IT) AND EXCHANGE, INTEGRATING INFORMATION WITH THE DELIVERY OF CARE TO IMPROVE THE WELLBEING OF INDIVIDUALS AND COMMUNITIES.

HEALTH INFORMATION EXCHANGE ONBOARDING AND OPERATIONS PROGRAM - SUPPORTS THE PARTICIPATION OF ELIGIBLE MEDICAID PROVIDERS IN BIDIRECTIONAL HEALTH INFORMATION EXCHANGE THROUGH HEALTH CURRENT. THE PROGRAM ENABLES ONBOARDING AND IMPLEMENTATION OF NEW PROVIDERS TO THE HIE AND SUPPORTS ONGOING MAINTENANCE AND OPERATIONS OF EXISTING PROVIDERS IN ALIGNMENT WITH MEDICAID INITIATIVES.
SOCIAL DETERMINANTS OF HEALTH - IN PARTNERSHIP WITH THE ARIZONA HEALTH CARE COST CONTAINMENT SYSTEM (AHCCCS), ARIZONA'S STATE MEDICAID AGENCY, HEALTH CURRENT DEVELOPED A CLOSED LOOP REFERRAL SYSTEM (CLRS) TO ADDRESS SOCIAL DETERMINANTS OF HEALTH (SDOH) NEEDS IN ARIZONA. IN CONCERT WITH AHCCCS, AND IN COLLABORATION WITH 2-1-1 ARIZONA, WHICH IS OPERATED BY SOLARI CRISIS & HUMAN SERVICES, THE TECHNOLOGY SOLUTION LAUNCHED IN 2021, AND MARKS A SIGNIFICANT MILESTONE IN SUPPORTING PROVIDERS, HEALTH PLANS, COMMUNITY-BASED ORGANIZATIONS (CBOS) AND COMMUNITY STAKEHOLDERS IN MEETING THE SOCIAL SERVICE NEEDS OF ARIZONANS.
DATA MANAGEMENT FEES - THESE ARE FEES THAT ORGANIZATIONS PAY FOR THE ABILITY TO PARTICIPATE IN THE HEALTH INFORMATION EXCHANGE (HIE). IN ADDITION, HIE PARTICIPANTS GAIN ACCESS TO EDUCATIONAL ACTIVITIES INCLUDING EVENTS AND WEBINARS, AS WELL AS UPDATES FROM THE HEALTH INFORMATION TECHNOLOGY (HIT) COMMUNITY.
