SUPPORTED BY FUNDING FROM THE MARGARET A. CARGILL PHILANTHROPIES (MACP) AND THE UCARE FOUNDATION, THE FOUNDATION CONTINUED ITS WORK ON THE CONNECTED COMMUNITIES FOR HEALTHY AGING PROJECT. TWO RURAL PILOTS WERE SELECTED TO CREATE INTEGRATED CARE SYSTEMS ACROSS AGING SERVICES, HEALTHCARE, AND COMMUNITY SUPPORTS. ASSISTANCE WAS PROVIDED TO PLAN, IMPLEMENT, AND EVALUATE OUTCOMES. OBJECTIVES INCLUDE: CONVENING AND ADVANCING LOCAL AND COLLABORATIVE WHOLE SYSTEM, WHOLE PERSON APPROACHES TO SERVING SENIORS IN COMMUNITY; EXPERIMENTING AND IMPROVING CARE OF MEDICARE ADVANTAGE ENROLLEES IN A LOCAL HEALTH PLAN IN COORDINATION WITH PROVIDERS OF PRIMARY CARE, ACUTE- AND POST-ACUTE CARE, HOME CARE, AND COMMUNITY-BASED SERVICES; CONNECTING CONSUMERS TO LOCAL RESOURCES FOR HEALTHIER AGING; IMPLEMENTING EFFECTIVE WORKFORCE STRATEGIES TO SUPPORT THIS COLLABORATIVE MODEL.
THE FOUNDATION OFFERS SCHOLARSHIPS TO HELP STAFF AND CAREGIVERS FROM OUR MEMBER ORGANIZATIONS ADVANCE THEIR PROFESSIONAL DEVELOPMENT. RECIPIENTS ARE SELECTED THROUGH A REVIEW PROCESS AND APPROVED BY THE BOARD OF DIRECTORS. IN 2021 SCHOLARSHIPS TOTALING $39,500 WERE AWARDED.
IN RESPONSE TO THE COVID-19 PANDEMIC, THE FOUNDATION PROVIDED MENTAL HEALTH SUPPORT TO LONG-TERM CARE FACILITIES THROUGH A SECOND ROUND OF INTERMEDIARY GRANTS FUNDED BY THE MARGARET A. CARGILL PHILANTHROPIES (MACP). THESE FUNDS ALSO ASSISTED RURAL ADULT DAY PROGRAMS WITH VIRTUAL PROGRAMMING WHEN IN-PERSON ACTIVITIES WERE PROHIBITED DUE TO COVID-19. ADDITIONAL MENTAL HEALTH SERVICES WERE PROVIDED BY THE FOUNDATION THROUGH A SUPPLEMENTAL GRANT FROM THE MINNESOTA DEPARTMENT OF HUMAN SERVICES.
