ONTRACK NURSING ASSISTANT TRAINING: THE FOUNDATION OPERATES ONTRACK, AN ASYNCHRONOUS ONLINE NURSING ASSISTANT TRAINING PROGRAM. THIS PROGRAM ADDRESSES THE DELAYS AND BARRIERS IN ACCESSING CNA TRAINING, INCREASES THE QUALITY OF THE CURRICULUM AND TRAINING, AND MEETS ALL TESTING REQUIREMENTS. IN 2023, 2,052 STUDENTS WERE TRAINED THROUGH ONTRACK.
WORKFORCE: THE FOUNDATION PROVIDED WORKFORCE GRANTS TO MANY SENIOR LIVING COMMUNITIES TO SUPPORT THEM IN RECRUITING AND RETAINING DIRECTCARE WORKFORCE EMPLOYEES.
CONNECTED COMMUNITIES: SUPPORTED BY FUNDING FROM THE MARGARET A. CARGILL PHILANTHROPIES (MACP) THE FOUNDATION CONTINUED ITS WORK ON THE CONNECTED COMMUNITIES FOR HEALTHY AGING PROJECT. THREE RURAL PILOTS WERE SUPPORTED IN ORDER TO CREATE INTEGRATED CARE SYSTEMS ACROSS AGING SERVICES, HEALTHCARE, AND COMMUNITY SUPPORTS. ASSISTANCE WAS PROVIDED TO PLAN, IMPLEMENT, AND EVALUATE OUTCOMES. OBJECTIVES INCLUDED: CONVENING AND ADVANCING LOCAL AND COLLABORATIVE WHOLE SYSTEM, WHOLE PERSON APPROACHES TO SERVING SENIORS IN COMMUNITY; EXPERIMENTING AND IMPROVING CARE OF MEDICARE ADVANTAGE ENROLLEES IN A LOCAL HEALTH PLAN IN COORDINATION WITH PROVIDERS OF PRIMARY CARE, ACUTE- AND POST-ACUTE CARE, HOME CARE, AND COMMUNITY-BASED SERVICES; CONNECTING CONSUMERS TO LOCAL RESOURCES FOR HEALTHIER AGING; IMPLEMENTING EFFECTIVE WORKFORCE STRATEGIES TO SUPPORT THIS COLLABORATIVE MODEL.
OTHER PROGRAM SERVICES:PROVIDED ADDITIONAL MENTAL HEALTH SUPPORTS TO CAREGIVERS THROUGH A PARTNERSHIP WITH WINONA STATE UNIVERSITY AND FUNDED BY A NEW GRANT FROM THE MN DEPARTMENT OF HEALTH.FUNDED CAREGIVER TRAINING ON ABUSE PREVENTION.DEVELOPED NEW RESOURCES FOR PROVIDERS TO EXPLORE AVAILABLE TECHNOLOGY TO IMPROVE SENIOR CARE OUTCOMES AND CREATE WORKFORCE EFFICIENCIES.ENGAGED IN RESEARCH TO IDENTIFY STRATEGIES TO IMPROVE DIVERSITY, EQUITY, INCLUSION AND BELONGING PRACTICES IN LONG-TERM CARE SETTINGS.FUNDED THE DEVELOPMENT OF TRAINED MEDICATION AIDE (TMA) CURRICULUM TO ADDRESS THE ACUTE SHORTAGE OF TMA'S IN MINNESOTA.
