SENIOR NUTRITION: THESE SENIOR PROGRAMS OPERATE MONDAY - FRIDAY FROM 9AM TO 5PM. WE PROVIDE DAILY MEALS, EXERCISE CLASSES, REFERRALS, ARTS AND CRAFTS, FIELD TRIPS AND SHOPPING TRIPS. THIS PROGRAM IS SUPPORTED WITH $33,612 IN GOVERNMENT GRANT CONTRIBUTIONS FOUND ON THE FORM 990, PAGE 9, PART VIII LINE 1(H).


OTHER PROGRAMS: THE SETTLEMENT HOUSE PROGRAM PROVIDES ADDITIONAL SUPPORT AND SERVICES TO OUR SENIORS, YOUTH AND FAMILIES IN THE SOUTHWEST COMMUNITY. THE EVICTION PREVENTION RENTAL ASSISTANCE PROGRAM IS ADMINISTERED BY THE COUNTY OF MONROE AND THE CITY OF ROCHESTER. SWAN WAS AWARDED THIS GRANT TO INTERVIEW INDIVIDUALS, DETERMINE ELIGIBILITY AND ADMINISTER RENTAL ARREARS TO PROPERTY OWNERS TO MAINTAIN HOUSING FOR MONROE COUNTY/CITY OF ROCHESTER RESIDENTS FACING EVICTION DUE TO CIRCUMSTANCES RELATED TO COVID.EMERGENCY SERVICES - SERVICES PROVIDED INCLUDE: FOOD AND CLOTHING REFERRALS, BUS TRANSPORTATION, UTILITY, RENTAL AND MORTGAGE ASSISTANCE, EMPLOYMENT AND JOB READINESS REFERRALS, FOOD VOUCHERS, SUMMER LUNCH PROGRAMS, HOLIDAY ASSISTANCE, FOOD, TOYS, AND CLOTHING, AND HOLIDAY DINNERS. THE GOAL IS TO INCREASE THE ABILITY OF LOW INCOME HOUSEHOLDS IN THE GREATER ROCHESTER AREA TO MEET THEIR BASIC FAMILY NEEDS AND INCREASE FINANCIAL STABILITY.THESE PROGRAMS ARE SUPPORTED WITH $56,229 IN GOVERNMENT GRANT CONTRIBUTIONS FOUND ON 990, PAGE 9, PART VIII LINE 1(H).


NEIGHBORHOOD COLLABORATIVE PROJECT:THE NEIGHBORHOOD COLLABORATIVE PROJECT IS AN EFFORT TO COLLABORATE ACROSS NEIGHBORHOOD COMMUNITY CENTERS AND SERVICE PROVIDERS TO STRENGTHEN THE COMMUNITY FROM WITHIN. THE COLLABORATIVE COORDINATES SERVICES AND RESOURCES TO IMPROVE INDIVIDUALS AND FAMILIES LIVELIHOODS AND, IN TIME, IMPROVE NEIGHBORHOOD CONDITIONS. THE NCP COMPRISES THREE ANCHOR AGENCIES. SWAN IS ONE OF THEM.THIS PROGRAM IS SUPPORTED WITH $86,290 IN CONTRIBUTIONS FROM MONROE COUNTY FOUND ON THE FORM 990, PAGE 9, PART VIII LINE 1(H).


PRESERVATION: UNDER THE NYS DIVISION OF HOUSING AND COMMUNITY RENEWAL PROGRAM WE PROVIDE HOUSING ASSISTANCE TO RENTERS AND HOMEOWNERS, AND COMMUNITY BASED ORGANIZED ACTIVITIES IN THE AREAS OF HOUSING ECONOMICS, COMMUNITY DEVELOPMENT AND PUBLIC SAFETY.THIS PROGRAM IS SUPPORTED WITH $220,220 IN GOVERNMENT GRANT AND ESL GRANT CONTRIBUTIONS FOUND ON THE FORM 990, PAGE 9, PART VIII LINE 1(H).
