HOME HEALTH OFFERS A COMBINATION OF CARE DELIVERED INSIDE THE HOME. THESE SERVICES ARE DIRECTED BY A PHYSICIAN. VNS PROVIDES CARE MANAGED BY A NURSE OR THERAPIST. ELDERLY, ADULTS AND CHILDREN ARE ALL ELIGIBLE ACCORDING TO THE INDIVIDUAL INSURANCE PLANS AND THE VNS GLADLY WORKS TO ACCEPT ALL PROVIDERS. ADDITIONALLY, THE ORGANIZATION PROVIDES CHARITY CARE TO PATIENTS WHO ARE UNABLE TO PAY. THE TOTAL NUMBER OF PEOPLE SERVED IN 2021 WAS 941.
THE AFFORDABLE CARE ACT (ACA) ESTABLISHED THE AUTHORITY FOR STATES TO DEVELOP AND RECEIVE REIMBURSEMENT FOR A SET OF HOME HEALTH SERVICES FOR THEIR STATE'S MEDICAID POPULATION WITH CHRONIC ILLNESS. HEALTH HOME SERVICES INCLUDE COMPREHENSIVE CARE MANAGEMENT, HEALTH PROMOTION, TRANSITIONAL CARE, PATIENT AND FAMILY SUPPORT, REFERRAL TO COMMUNITY AND SOCIAL SUPPORT SERVICES, AND USE OF HIT TO LINK SERVICES. TO BE ELIGIBLE UNDER NEW YORK STATE GUIDELINES, A PATIENT MUST HAVE TWO CHRONIC CONDITIONS, OR ONE SINGLE QUALIFYING CONDITION (HIV/AIDES OR SMI). THE HEALTH HOME SERVED 941 PATIENTS IN 2021.
