AGING IN AMERICA SUPPORTIVE SERVICES, INC. PROVIDES CASE MANAGEMENT AND SERVICE COORDINATION TO LOW-INCOME SENIORS LIVING AT HAVERSTRAW PLACE IN ROCKLAND COUNTY. THE FOCUS OF THESE SERVICES IS ON MAINTAINING SELF-RELIANCE, INDEPENDENCE AND ENHANCING EACH PERSONS SENSE OF FREEDOM AND DIGNITY. SERVICES INCLUDE HOME ASSESSMENT, INFORMATION AND REFERRAL, BENEFITS ENTITLEMENT AND COUNSELING, ASSISTANCE WITH TRANSPORTATION AND COORDINATION OF NEEDED SERVICES. AN ON-SITE SERVICE COORDINATOR ASSISTS RESIDENTS WITH ACCESSING THESE SERVICES.
