THE ASSOCIATION PROVIDES EDUCATIONAL, VOCATIONAL, AND RESIDENTIAL SERVICES FOR INDIVIDUALS WHO ARE DEVELOPMENTALLY DISABLED.

RESIDENTIAL SERVICES PROGRAM (PEOPLE SERVED - 71, DAYS SERVED 365/YEAR): PROVIDES HEALTHY LIVING ENVIRONMENT TO INDIVIDUALS WHO HAVE INTELLECTUAL AND DEVELOPMENTAL DISABILITIES. THE ASSOCIATION'S RESIDENTIAL PROGRAM IS COMPRISED OF INDIVIDUAL RESIDENTIAL ALTERNATIVE (IRA) APARTMENTS AND HOUSES IN QUEENS, NEW YORK. THE RESIDENTIAL PROGRAMS OFFER PERSONS WITH DEVELOPMENTAL DISABILITIES THE OPPORTUNITY TO LIVE AS INDEPENDENTLY AS POSSIBLE WITHIN THEIR COMMUNITY. EVERY PERSON'S INPUT IS ENCOURAGED IN DEVELOPING THE PHYSICAL SETTING AND SOCIAL MAKE-UP OF THEIR HOME.


DAY SERVICES PROGRAMS (PEOPLE SERVED - 139, DAYS SERVED 260/YEAR):THE DAY HABILITATION PROGRAM SERVES PARTICIPANTS FROM AGES 17 TO SENIOR TO HELP INDIVIDUALS REACH THEIR POTENTIAL THROUGH EDUCATIONAL, VOCATIONAL, AND RECREATIONAL ACTIVITIES DESIGNED TO FOSTER PRODUCTIVITY, EMPOWERMENT, INDEPENDENCE, AND COMMUNITY INTEGRATION. VOCATIONAL SERVICES PROGRAM SERVES INDIVIDUALS BY PROVIDING VOCATIONAL TRAINING AND SUPPORT SERVICES SO THEY MIGHT BE GAINFULLY EMPLOYED. THE PRE-VOCATIONAL TRAINING PROGRAM IS A SELF-PACED, PERSON CENTERED PROGRAM INDIVIDUALLY DESIGNED TO ASSIST THE INDIVIDUAL PLAN TO PREPARE FOR THE WORLD OF WORK. IN ADDITION, WE OFFER A SUPPORTED EMPLOYMENT PROGRAM THAT WORKS WITH AN INDIVIDUAL IN IDENTIFYING EMPLOYMENT OPPORTUNITIES BY USING A PERSON-CENTERED PLAN PAYING CLOSE ATTENTION TO INDIVIDUAL GOALS. PROVIDES RECREATIONAL PROGRAMS TO INDIVIDUALS OF ALL AGES WITH IDD/ASD. THE ASSOCIATION OFFERS A VARIETY OF RECREATIONAL AND SOCIAL ACTIVITIES INCLUDING: UNIFIED RECREATION AND RESPITE PROGRAM ARE COMPRISED OF 2 COMPONENTS: FRIDAY NIGHT SOCIAL FOR ADULTS AND WEEKEND RESPITE PROGRAM FOR CAREGIVERS. THESE PROGRAMS ARE INTENDED TO PROVIDE OPPORTUNITIES FOR SOCIAL INTERACTION, COMMUNITY INTEGRATION, PHYSICAL ACTIVITY AND PLAY, AND RESPITE FOR CAREGIVERS. OUR FRIDAY NIGHT SOCIAL PROVIDES AN ENVIRONMENT WHERE INDIVIDUALS CAN SOCIALIZE AND DEVELOP AND MAINTAIN FRIENDSHIPS. THE PROGRAM MEETS EACH FRIDAY NIGHT FROM SEPTEMBER THROUGH JUNE. ACTIVITIES INCLUDE ARTS AND CRAFTS, BINGO, CARDS, MOVIES, KARAOKE, TALENT SHOWS AND HOLIDAY PARTIES. THE RESPITE PROGRAM IS TARGETED AT CARE GIVERS WHO PROVIDE CARE FOR INDIVIDUALS IN THEIR HOME. MEANINGFUL ACTIVITIES PROVIDED FOR THE INDIVIDUAL DURING RESPITE PERIODS INCLUDE COMMUNITY EXPLORATION, INDOOR SOCIAL ACTIVITIES, INCREASED PEER INTERACTION AND OPPORTUNITIES TO DEVELOP FRIENDSHIPS.


FAMILY SUPPORT SERVICES PROGRAM (PEOPLE SERVED - 79, DAYS SERVED 260/YEAR): FAMILY SUPPORT IS A STATE FUNDED CASE MANAGEMENT PROGRAM. CASE MANAGERS ASSIST INDIVIDUALS WITH DISABILITIES AND THEIR CAREGIVERS IN ACCESSING SERVICES THAT HAVE POSITIVE IMPACTS ON INDIVIDUAL LIVES. THIS INCLUDES IN-HOME SERVICES, EDUCATION, VOCATIONAL PROGRAMS, DAY PROGRAMS, RECEREATION, PERSONAL LIFE-NEEDS, TRANSPORTATION, MEDICAL ACCESS, ALONG WITH BENEFITS AND ENTITLEMENTS. CASE MANAGERS ALSO ASSIST INDIVIDUALS WITH THE PROCESS REQUIRED TO ENROLL IN THE MEDICAID WAIVER SO THEY CAN ACCESS HCBS SERVICES.PROVIDES OUTREACH, PUBLIC AWARENESS, FAMILY AND INDIVIDUAL COUSELING, AND SERVICE COORDINATION TO INDIVIDUALS AND FAMILIES OF PEOPLE WHO ARE NEUROLOGICALLY IMPAIRED AND WHO DON'T HAVE MEDICAID.


COMMUNITY HABILITATION:COMMUNITY HABILITATION IS A MEDICAID FUNDED PROGRAM OPERATED UNDER OPWDD TO PROVIDE ONE-TO-ONE TRAINING TO PERSONS WITH INTELLECTUAL / DEVELOPMENTAL DISABILITIES TO DEVELOP OR ENHANCE THE SKILLS NEEDED TO LIVE MORE INDEPENDENTLY IN THEIR HOMES OR IN THE COMMUNITY.
