THE ORGANIZATION WAS ESTABLISHED TO PROVIDE RESIDENTIAL SERVICES TO DEVELOPMENTALLY DISABLED INDIVIDUALS, INCLUDING COMMUNITY LIVING ARRANGEMENTS. THE ORGANIZATION ALSO OPERATES A DAY PROGRAM WHEREBY INDIVIDUALS WITH CERTAIN DISABILITIES CAN BECOME PRODUCTIVE MEMBERS OF SOCIETY.

COMMUNITY LIVING ARRANGEMENTS (CLA) ARE LICENSED HOMES THAT PROVIDE SUPPORT TO APPROXIMATELY 125 INDIVIDUALS. WE OFFER 30 CLA HOMES THAT INCLUDE SERVICES DESIGNED TO ADDRESS AN INDIVIDUAL'S MEDICAL, PSYCHIATRIC AND BEHAVIORAL CHALLENGES. EACH PERSON WHO LIVES IN A CLA IS A PART OF AN INTERDISCIPLINARY TEAM THAT MEETS REGULARLY TO IDENTIFY AND DISCUSS HOW TO MEET ALL OF THE PARTICIPANTS' WISHES, GOALS AND CHALLENGES INTERMEDIATE CARE FACILITY (ICF) ARE HOMES THAT PROVIDE HEALTH RELATED CARE AND SERVICES TO INDIVIDUALS WHO DO NOT REQUIRE THE DEGREE OF CARE THAT SKILLED NURSING FACILITIES PROVIDE. WE OFFER 2 ICF HOMES FOR 8 INDIVIDUALS. IN HOME SUPPORT (IHS) ARE PROFESSIONAL SUPPORT SERVICES THAT WE PROVIDE TO INDIVIDUALS IN A PERSON'S HOME. WE ARE CURRENTLY SUPPORTING 9 INDIVIDUALS.
DAY SUPPORT OPTIONS EMPHASIZES A COMMUNITY EXPERIENCE AND PARTICIPATION AS A VEHICLE FOR PERSONAL INDEPENDENCE. APPROXIMATELY 134 INDIVIDUALS CAN CHOOSE TO PARTICIPATE IN A VARIETY OF COMMUNITY-BASED ACTIVITIES AND EVENTS. OPPORTUNITIES RANGE FROM SOCIAL AND RECREATIONAL ACTIVITIES TO WORKING IN THE COMMUNITY. PERSONS WITH BEHAVIORAL CHALLENGES OFTEN HAVE REWARDING AND SUCCESSFUL EXPERIENCES IN THIS PROGRAM.
