A PRIVATE, NON-PROFIT OFFERING INDEPENDENT LIVING PROGRAMS DESIGNED TO TEACH INDEPENDENT LIFE SKILLS TO DEVELOPMENTALLY DISABLED INDIVIDUALS AGE EIGHTEEN AND OVER. THE GOAL IS TO TEACH THE LIFE SKILLS NEEDED SO THESE INDIVIDUALS MAY LIVE INDEPENDENTLY OUT IN THE COMMUNITY AND TO MAINTAIN THIS INDEPENDENCE THROUGH SEVERAL SUPPORT SERVICES.

RESIDENTIAL AND RESIDENTIAL TRANSITION PROGRAMS SUPPORTING INDIVIDUALS FROM AROUND THE WORLD. CHAPEL HAVEN SHLEIFER CENTER IS HIGHLY REGARDED IN SERVING INDIVIDUALS WITH A VARIETY OF DEVELOPMENTAL DISABILITIES TO LEARN TO LIVE INDEPENDENTLY AND WITH MINIMAL SUPPORTS IN OUR EXTENDED COMMUNITY. MOVING FROM A 24 HOUR SUPERVISED PROGRAM TO THE SURROUNDING COMMUNITY, THESE INDIVIDUALS WILL LIVE IN THEIR OWN APARTMENTS, MANAGE THEIR DAILY ROUTINES, WORK IN A VARIETY OF JOBS AND ENJOY A VIBRANT SOCIAL NETWORK.
CHAPEL HAVEN SCHLEIFER CENTER'S DAY PROGRAM PROVIDES A CONTINUUM OF SUPPORT SERVICES FOR THE MEN AND WOMEN WHO CHOOSE TO STAY IN OUR COMMUNITY. WE CURRENTLY SUPPORT APPROXIMATELY 150 ADULTS WHO ARE LIVING OUT THEIR DREAMS OF HAVING THEIR OWN PLACE, EMPLOYMENT, AND ENJOYING THEIR CIRCLE OF FRIENDS. SERVICES ARE STRUCTURED TO MEET THE NEEDS OF EACH INDIVIDUAL TO ENSURE THEY MAINTAIN THEIR INDEPENDENCE IN OUR COMMUNITY. SERVICES INCLUDE AND ARE NOT LIMITED TO, BANKING AND FINANCING, NUTRITION, HEALTH AND WELLNESS, RECREATION AND LEISURE, MEDICAL TRANSPORTATION, ETC.
