PROVIDES SPECIALIZED SERVICES TO ASSIST INDIVIDUALS AND THEIR FAMILIES IN OVERCOMING THE CHALLENGES OF DEVELOPMENTAL DISABILITIES, MENTAL ILLNESS, AND MEDICAL CONDITIONS.
Describe the Organization's Program Activity:
ADULT FOSTER CARE PROGRAM IS A NON-CONTRACT FEE-FOR-SERVICE PROGRAM FUNDED BY THE COMMONWEALTH OF MASSACHUSETTS FOR PROVIDING QUALIFIED INDIVIDUALS DAILY ASSISTANCE AND PERSONAL CARE AT HOME BY CAREGIVERS. MASSHEALTH PAYS A DAILY RATE PER MEMBER.
OTHER PROGRAMS INCLUDE INDIVIDUAL SUPPORT SERVICES, PLACEMENT SERVICES, TRANSPORTATION SERVICES AND COMMERCIAL REAL ESTATE ACTIVITIES.
RESIDENTIAL SUPPORT SERVICE PROGRAM PROVIDES COMPREHENSIVE SUPPORT TO PERSONS WITH DEVELOPMENTAL DISABILITIES IN THE COMMUNITY IN A 24-HOUR GROUP LIVING ARRANGEMENT.
