PRIMARY CARE SERVICES:MEDICAL CARE OF BOSTON MANAGEMENT CORP D/B/A BETH ISRAEL LAHEY HEALTH PRIMARY CARE A/K/A AFFILIATED PHYSICIANS GROUP (APG/BILHPC) HAS APPROXIMATELY FORTY PHYSICIAN OFFICES THAT SPECIALIZE IN THE DELIVERY OF PRIMARY CARE AND FAMILY MEDICINE IN TWENTY THREE COMMUNITIES IN AND AROUND THE BOSTON, MASSACHUSETTS AREA. PRIMARY CARE PHYSICIANS (PCPS) ARE COMMUNITY BASED MEDICAL DOCTORS WHO GENERALLY PROVIDE THE FIRST CONTACT FOR A PATIENT WITH A NON-EMERGENT UNDIAGNOSED HEALTH CONCERN, AS WELL AS CONTINUING CARE FOR A VARIETY OF MEDICAL CONDITIONS, KEEPING CARE IN THE COMMUNITY WHEN IT IS APPROPRIATE. WHEN ADVANCED CARE IS NEEDED, PATIENTS HAVE SEAMLESS ACCESS TO LEADING-EDGE TREATMENT FROM BETH ISRAEL DEACONESS MEDICAL CENTER, A WORLD RENOWNED TERTIARY CARE ACADEMIC MEDICAL CENTER, AND AN AFFILIATE OF APG/BILHPC OR BETH ISRAEL DEACONESS HOSPITAL - NEEDHAM, BETH ISRAEL DEACONESS HOSPITAL - MILTON, BETH ISRAEL DEACONESS HOSPITAL - PLYMOUTH OR ANNA JAQUES HOSPITAL FOR CARE MORE LOCALLY IN THE PATIENTS' COMMUNITIES DURING THE FISCAL YEAR COVERED BY THIS RETURN, APG PHYSICIANS HAD APPROXIMATELY 394,000 PRIMARY CARE VISITS.
EXTENDED CARE COMMUNITY PRACTICE AND HOME CARE:THE EXTENDED CARE COMMUNITY PROGRAM (ECCP) IS COMPRISED OF NINE PHYSICIANS AND THIRTEEN NURSE PRACTITIONERS WHO SPECIALIZE IN THE CARE OF PATIENTS IN THE POST-ACUTE ENVIRONMENT INCLUDING SKILLED NURSING, ASSISTED LIVING AND REST HOME FACILITIES. THESE CLINICIANS PROVIDE CONTINUITY OF CARE FOR APG/BILHPC PATIENTS WHEN THEY ARE IN NEED OF POST-ACUTE CARE. THEIR EXCLUSIVE FOCUS IS PROVIDING THE BEST PATIENT CARE FOR THE PATIENT DURING A POST-ACUTE STAY. ECCP PROVIDERS ARE LOCATED AT 43 POST-ACUTE FACILITIES, PRIMARILY NEAR OR WITHIN THE PATIENT SERVICE AREAS OF BETH ISRAEL DEACONESS MEDICAL CENTER AND ITS AFFILIATED BILH HOSPITALS, AND ON A FULL-TIME BASIS AND CAN QUICKLY FOLLOW-UP ON PATIENT CARE ISSUES AND PROGRESS. ECCP PROVIDERS COMMUNICATE DIRECTLY WITH PATIENTS, FAMILY MEMBERS, PCPS AND DIRECT CARE STAFF AT THE FACILITIES WHERE PATIENTS ARE PHYSICALLY BASED ON A ROUTINE BASIS. THEY DO NOT SEE PATIENTS IN THE PRIMARY CARE OFFICES SO THEIR SOLE FOCUS IS TO TREAT AND COORDINATE TREATMENT OF APG PATIENTS WHO HAVE BEEN ADMITTED TO A POST-ACUTE CARE FACILITY WHILE COMMUNICATING DIRECTLY WITH AN APG PCP AND/OR SPECIALISTS DURING THEIR STAY AND UPON DISCHARGE BACK TO THE COMMUNITY. DURING THE FISCAL YEAR COVERED BY THIS RETURN, THE ECCP PROGRAM INCLUDED APPROXIMATELY 43,500 PATIENT VISITS.
SPECIALTY CARE:APG/BILHPC NEUROLOGY SPECIALISTS LOCATED IN NEEDHAM, MASSACHUSETTS PROVIDE OFFICE BASED LOCAL COMMUNITY ACCESS TO SPECIALTY CARE. WHEN ADVANCED CARE IS NEEDED, PATIENTS ARE REFERRED TO THE BETH ISRAEL MEDICAL CENTER OR OTHER AFFILIATED BILH HOSPITAL. DURING THE FISCAL YEAR COVERED BY THIS RETURN, APG/BILHPC SPECIALISTS HAD APPROXIMATELY 2,900 PATIENT VISITS RELATED TO NEUROLOGY.IN ADDITION, APG/BILHPC PROVIDES SUPPORT SERVICES TO CERTAIN BILH EMPLOYED PHYSICIANS IN PLYMOUTH, MASSACHUSETTS AND NEWBURYPORT, MASSACHUSETTS AND THE OTHER COMMUNITIES SERVED BY BID-PLYMOUTH AND ANNA JAQUES HOSPITALS. THESE SERVICES SUPPORT THE PROVISION OF OFFICE BASED LOCAL COMMUNITY ACCESS TO SPECIALTY CARE IN THE AREAS OF SPINE, NEUROLOGY AND NEUROSURGERY, OBSTETRICS AND GYNECOLOGY, ENDOCRINOLOGY AND GERIATRICS. WHEN ADVANCED CARE IS NEEDED, PATIENTS ARE REFERRED TO THE BETH ISRAEL MEDICAL CENTER OR OTHER AFFILIATED BILH COMMUNITY HOSPITALS.
