INDIVIDUAL SUPPORTS- PROVIDE SUPPORTS TO 155 INDIVIDUALS SERVED LIVING IN THEIR OWN HOMES INDEPENDENTLY. THE AMOUNT OF HOURS PER WEEK OF SERVICE IS BASED ON A PARTICULAR INDIVIDUAL'S NEEDS FOR SUPPORTS.
