OUR HEALTH CENTERS ARE STAFFED BY A MULTI-DISCIPLINARY TEAM OF HEALTH CARE PROFESSIONALS WHO WORK COLLABORATIVELY TO PROVIDE QUALITY, PATIENT-CENTERED CARE TO IMPROVE INDIVIDUAL HEALTH AND THE HEALTH OF THE COMMUNITY. OUR MEDICAL STAFF INCLUDES PHYSICIANS, ADVANCED-PRACTICE CLINICIANS, NURSES, BEHAVIORAL HEALTH PROVIDERS, MEDICAL ASSISTANTS AND HEALTH COACHES. SPLG COORDINATES CONTINUITY OF PATIENT CARE TO SPECIALISTS THROUGH AGREEMENTS, AFFILIATIONS AND REFERRALS TO PARTNERING PROVIDERS.
