THE ORGANIZATION IS A FEDERALLY QUALIFIED HEALTH CENTER THAT PROVIDES MEDICAL, DENTAL, VISION, AND MENTAL HEALTH CARE, AS WELL AS EDUCATION, OUTREACH, AND PREVENTION SERVICES.

EAST VALLEY COMMUNITY HEALTH CENTER (EVCHC) IS DESIGNATED AS A FEDERALLY QUALIFIED HEALTH CENTER. EVCHC OPERATES EIGHT SITES IN WEST COVINA, POMONA, COVINA AND LA PUENTE. HEALTH CARE PROVIDED AT EVCHC INCLUDES: FAMILY MEDICINE, PEDIATRICS, PRE AND POST-NATAL CARE, WOMEN'S HEALTH CARE, INCLUDING FAMILY PLANNING AND CANCER PREVENTION, HIV/AIDS COMPREHENSIVE CARE, DENTAL CARE, VISION CARE AND MENTAL HEALTH CARE. EVCHC PROVIDED SERVICES TO 24,577 PATIENTS WHO RECEIVED OVER 129,000 VISITS (IN-PERSON AND VIRTUAL). NUTRITION, CARE COORDINATION AND CARE MANAGEMENT ARE AN INTEGRAL PART OF OUR CHRONIC DISEASE MANAGEMENT SERVICES. PHARMACY SERVICES ARE AVAILABLE AT ALL OF OUR HEALTH CENTERS. EVCHC OUTREACH EDUCATION SERVICES FOCUS ON PREVENTION AND TESTING OF HIV FOR VARIOUS AT-RISK POPULATIONS. THIS YEAR, SINCE THE COVID-19 VACCINE INCEPTION EVCHC HAS PROVIDED 58,000 DOSES TO OUR PATIENTS AND COMMUNITY AT LARGE. WE ALSO PROVIDE COVID-19 EDUCATION THROUGH SOCIAL MEDIA AND COMMUNITY OUTREACH. WE CONTINUE TO SERVE THE HOMELESS POPULATION ON THE STREET AND HOMELESS SHELTERS. OUR TEAM OF ENROLLMENT SPECIALISTS PROVIDE ENROLLMENT ASSISTANCE AT EACH OF OUR SITES.
