SUN LIFE HEALTH HAS BEEN SERVING OUR COMMUNITIES FOR OVER 46 YEARS. SERVING NEARLY 50,000 PATIENTS ANNUALLY IN 14 LOCATIONS THROUGHOUT MARICOPA AND PINAL COUNTIES, SUN LIFE HEALTH OFFERS COMPREHENSIVE PRIMARY CARE SERVICES THAT INCLUDE DIAGNOSIS, TREATMENT AND FOLLOW UP CARE TO ADDRESS THE CHRONIC HEALTH CARE CONDITIONS AFFECTING THE TARGET POPULATION IN THE SERVICE AREA. PRIMARY CARE SERVICES DELIVERED TO THE TARGET POPULATION FOCUS ON IMPROVING HEALTH OUTCOMES TO ADDRESS HIGH RATES FOR CANCERS, CARDIOVASCULAR DISEASE, DIABETES, OBESITY AND ASTHMA IDENTIFIED FOR THE TARGET POPULATION. SUN LIFE HEALTH SERVES PATIENTS THROUGHOUT THE LIFE CYCLE (FORM BIRTH TO END OF LIFE). OUR PEDIATRIC OFFICE OFFERS WELLNESS CHECKS, IMMUNIZATIONS AS WELL AS EDUCATION AND PREVENTIVE HEALTH SERVICES TO THE PATIENT POPULATION AGE 18 AND YOUNGER. SUN LIFE HEALTH ALSO IMPLEMENTS OB/GYN SERVICES IN FOUR LOCATIONS LOCATED IN APACHE JUNCTION, CASA GRANDE, CHANDLER AND MARICOPA. SERVICES PROVIDED AT THE OB/GYN OFFICES INCLUDE HEALTH EDUCATION, ULTRASOUND, WELLNESS WOMEN EXAMS, PAP TESTS, FAMILY PLANNING SERVICES, PREGNANCY TESTS, BREAST EXAMS AS WELL AS PRENATAL CARE AND LABOR AND DELIVERY. THESE SERVICES ADDRESS HEALTH DISPARITIES RELATED TO NEWBORN AND MATERNAL HEALTH IN THE COMMUNITIES WE SERVE. AS THE ONLY RYAN WHITE PART A PRIMARY CARE SERVICE PROVIDER IN PINAL COUNTY, SUN LIFE ALSO PROVIDES MEDICAL SERVICES THAT FOCUS ON MANAGING THE COMPLEX HEALTH CARE NEEDS OF INDIVIDUALS WHO HAVE HIV/AIDS AND ALSO AID IN THE PREVENTION OF HIV BY OFFERING HIV PREP SERVICES. SUN LIFE HEALTH HAS BEEN PROACTIVE IN THE FIGHT AGAINST COVID-19 BY OFFERING EDUCATION, TESTING AND VACCINATION SERVICES IN MANY COMMUNITIES THAT WOULD OTHERWISE GO WITHOUT. WE HAVE PARTNERED WITH SCHOOLS, MUNICIPALITIES AND EVEN PRIVATE BUSINESSES TO INCREASE TESTING AND VACCINATION RATES THROUGHOUT PINAL COUNTY. FOR THE FIRST TIME, SUN LIFE HEALTH WAS ABLE TO OFFER IN-HOME VACCINATIONS TO SOME OF THE POPULATIONS MOST VULNERABLE HOMEBOUND RESIDENTS. TO DATE SUN LIFE HEALTH HAS ADMINISTERED 47,000 VACCINES AND THAT NUMBER CONTINUES TO GROW DAILY.
PHARMACY SERVICES ARE AVAILABLE TO THE TARGET POPULATION AT ALL HEALTH CENTER LOCATIONS. TO FACILITATE ACCESS TO THESE SERVICES, SUN LIFE HEALTH IMPLEMENTS FULL-SERVICE PHARMACIES ONSITE AT FAMILY PRACTICE LOCATIONS IN APACHE JUNCTION, CASA GRANDE, ELOY, FLORENCE, MARICOPA, ORACLE, SAN MANUEL AND AT THE NEW CENTER FOR WOMEN IN CASA GRANDE. SUN LIFE'S PARTICIPATION IN THE 340B PROGRAM ALLOWS US TO OFFER PHARMACEUTICALS AT AN AFFORDABLE PRICE TO OUR CONSUMERS. OUR CLINICAL PHARMACIST TEAM ASSISTS PATIENTS WITH MEDICATION MANAGEMENT. AT SUN LIFE HEALTH, THE PHARMACISTS ARE AN INTEGRAL PART OF PATIENT CARE.
SUN LIFE OPERATES A FAMILY DENTISTRY OFFICE ON SITE AT THE MAIN FAMILY PRACTICE OFFICE IN CASA GRANDE. THE ORGANIZATION ALSO OPERATES A PEDIATRIC DENTAL PRACTICE IN CASA GRANDE. DENTAL SERVICES AVAILABLE AT THESE LOCATIONS INCLUDE ORAL HEALTH SCREENING, FILLINGS, EXTRACTIONS, CROWNS, BRIDGES, ROOT CANALS AND FULL AND PARTIAL DENTURES. SUN LIFE ALSO IMPLEMENTS A COMMUNITY DENTAL PROGRAM THROUGH A CONTRACT WITH ARIZONA DEPARTMENT OF HEALTH SERVICES OFFICE OF ORAL HEALTH TO PROVIDE HEALTH SCREENING AND DENTAL SEALANTS AT PINAL COUNTY SCHOOLS. WE RECENTLY STARTED A PEDIATRIC DENTAL RESIDENCY PROGRAM IN PARTNERSHIP WITH NYU LANGONE, GROWING OUR PEDIATRIC DENTAL TEAM. THESE SERVICES HELP TO IMPROVE ORAL HEALTH OUTCOMES FOR BOTH ADULTS AND CHILDREN IN SUN LIFE'S SERVICE AREA.
ALL HEALTH CENTER LOCATIONS HAVE INTEGRATED BEHAVIORAL HEALTH SERVICES AVAILABLE ONSITE. CONSULTATIONS WITH A BEHAVIORAL HEALTH CONSULTANT ARE AVAILABLE TO PATIENTS THAT PRESENT WITH BEHAVIORAL HEALTH ISSUES DURING THEIR MEDICAL VISIT. BEHAVIORAL HEALTH CONSULTANTS WORK COLLABORATIVELY WITH PHYSICIANS TO COUNSEL AND EDUCATE PATIENTS ABOUT MODIFYING BEHAVIORS THAT PROMOTE POOR HEALTH OUTCOMES AND/OR WORSEN THEIR CHRONIC CONDITIONS. OUR INTEGRATED BEHAVIORAL HEALTH TEAM HELP PEOPLE LIVE HEALTHIER LIVES BY OFFERING MEDICATION ASSISTED THERAPIES, TOBACCO CESSATION, WEIGHT- LOSS, ALTERNATIVE PAIN MANAGEMENT, AND MANY OTHER HEALTH EDUCATION PROGRAMS.
