SERVING NEARLY 50,000 PATIENTS ANNUALLY IN 14 LOCATIONS THROUGHOUT MARICOPA AND PINAL COUNTIES, SUN LIFE HEALTH OFFERS COMPREHENSIVE PRIMARY CARE SERVICES THAT INCLUDE DIAGNOSIS, TREATMENT AND FOLLOW UP CARE TO ADDRESS THE CHRONIC HEALTH CARE CONDITIONS. PRIMARY CARE SERVICES DELIVERED TO THE TARGET POPULATION FOCUS ON IMPROVING HEALTH OUTCOMES TO ADDRESS HIGH RATES FOR CANCERS, CARDIOVASCULAR DISEASE, DIABETES, OBESITY AND ASTHMA. SUN LIFE HEALTH SERVES PATIENTS THROUGHOUT THE LIFE CYCLE (FORM BIRTH TO END OF LIFE).
