SUPPORT ONGOING ASSISTANCE WITH LIFE MANAGEMENT SKILLS FOR THOSE PEOPLE AND ASSIST IN ACCESSING APPROPRIATE SERVICES TO MEET THEIR NEEDS IN THE COMMUNITY.
