TO DATE, WE HAVE PROVIDED PROFESSIONAL DEVELOPMENT AND DELIVERED PRESENTATIONS TO EIGHT ORGANIZATIONS, REACHING 100+ ATTENDEES. AS A RESULT OF OUR OUTREACH, WE HAVE BEEN ABLE TO PROVIDE ADVOCACY SERVICES TO APPROXIMATELY 25 INDIVIDUALS AND THEIR FAMILIES ON HOW TO ACCESS AND APPLY FOR RESOURCES SUCH AS GOVERNMENT FUNDING, EDUCATING FAMILIES ON THE PUNS LIST AND HOW TO ACCESS, PLANNING THE INDIVIDUAL'S TRANSITION PLAN POST HIGH SCHOOL, RESEARCHING AND TOURING RESIDENTIAL FACILITIES FOR PLACEMENT, EMERGENCY CONTACT AND CARE COORDINATION SERVICES FOR SENIORS AND INTERVENTIONS THROUGH THE ILLINOIS MEDICAID WAIVER PROGRAMS.
