SERVICES INCLUDE THE DIAGNOSIS AND TREATMENT OF ILLNESS AND INJURY, CHRONIC DISEASE MANAGEMENT, SUBSTANCE USE DISORDER TREATMENT, HEALTH EDUCATION, AND PREVENTIVE CARE. SERVICES ALSO INCLUDE MATERNITY SUPPORT, OBSTETRICS, AND PREVENTATIVE MEDICINE SPECIALISTS. ENABLING, RESOURCE BROKERING, AND HOMELESS OUTREACH SERVICES ALSO ARE OFFERED.
