CHRISTIAN CARE MESA, INC. DBA FELLOWSHIP SQUARE-MESA IS SET AGAINST THE SPECTACULAR BACKDROP OF THE WORLD-FAMOUS SUPERSTITION MOUNTAINS. HERE OUR RESIDENTS ENJOY AN ENCHANTING, SOUTHWEST RESORT STYLE SENIOR LIVING COMMUNITY, AN INVITING DESERT OASIS WITH LUSH LAWNS, HEATED POOL AND SPA, PATIOS, KOI POND, AND SHADED RAMADAS FOR THEIR RECREATION CONVENIENCE. THEY ENJOY THE FREEDOM TO EXPLORE AND DISCOVER THE THINGS THEY ARE PASSIONATE ABOUT, AS WELL AS A PLACE WHERE FUN, FRIENDSHIP, AND GREAT FOOD ABOUND.FELLOWSHIP SQUARE-MESA CONSISTS OF 194 ONE-BEDROOM AND TWO-BEDROOM SUITES AND 42 ASSISTED LIVING UNITS INCLUDING 12 STUDIO, 24 ONE BEDROOM AND 6 DELUXE ONE-BEDROOM MODELS. OUR INDEPENDENT (CONT'D ON SCH. O) LIVING RESIDENTS ENJOY AT LEAST 25 MEALS PER MONTH, SEMI-MONTHLY HOUSEKEEPING, 24-HOUR SECURITY, AND PRIVATE VAN AND BUS SCHEDULED TRANSPORTATION TO GROCERY AND OTHER SHOPPING, MEDICAL APPOINTMENTS, RESTAURANTS, AND OTHER VENUES AROUND TOWN. ASSISTED LIVING RESIDENTS HAVE SIMILAR SERVICES BUT HAVE ACCESS TO SHARED CAREGIVERS AND 3 MEALS PER DAY.DEPENDING ON OUR RESIDENTS' NEEDS WE PROVIDE A VARIETY OF PROGRAMS TO ENRICH, ENCOURAGE AND PROMOTE A LIFESTYLE TO SERVE RESIDENTS NOW AND IN THE FUTURE. NEARBY SHOPPING, CULTURAL, MEDICAL, AND LIFE-LONG LEARNING OPPORTUNITIES ARE COMPLETED ONSITE BY AN ARRAY OF VOLUNTEER, SOCIAL, FITNESS, SPIRITUAL, AND WELLNESS ACTIVITIES. THE NATIONALLY KNOWN SENIOR FIT WELLNESS PROGRAM FEATURES A PERSONAL TRAINER TO ENSURE SUCCESS; THE ASSISTED LIVING FITNESS CENTER HAS A STUNNING VIEW OF THE WORLD-FAMOUS SUPERSTITION MOUNTAINS. WE ALSO PROVIDE ASSISTED LIVING SERVICES TO UP TO 65 SUBSIDIZED HOUSING RESIDENTS NEXT DOOR. FELLOWSHIP SQUARE-MESA HAS A ROBUST ACTIVITY PROGRAM WITH A VARIETY OF EVENTS ON CAMPUS AND CULTURALLY ENRICHING ACTIVITIES OFF-CAMPUS TO PROVIDE RESIDENTS WITH LOTS OF OPPORTUNITIES FOR FELLOWSHIP AND FUN! FELLOWSHIP SQUARE-MESA ADMINISTERS THE "M4-ALTCS ASSISTED LIVING PROGRAM" THAT FEATURES SPACIOUS ONE-BEDROOM APARTMENTS WITH FULL SIZED APPLIANCES IN EACH KITCHEN AND WALK-IN SHOWERS FOR RESIDENT SAFETY. THE RESIDENTS ARE NEVER ASKED TO SHARE THEIR LIVING QUARTERS WITH ANYONE ELSE (DOUBLE OCCUPANCY IS A COMMON PRACTICE IN ALTCS BUILDINGS). THE DEDICATED STAFF PROVIDES SPECIALIZED ASSISTED LIVING SERVICES 24/7 TO MEET THE RESIDENT'S INDIVIDUAL PERSONAL CARE REQUIREMENTS. THIS SERVICE IS DESIGNED TO PREVENT PREMATURE INSTITUTIONALIZATION OF FRAIL ELDERLY RESIDENTS ALLOWING THEM TO RECEIVE THE SUPPORT THEY REQUIRE, WHILE REMAINING IN AN ENVIRONMENT WHERE THEY ARE COMFORTABLE.DURING 2019 FELLOWSHIP SQUARE-MESA STARTED ADDING SMART APARTMENTS TO THE AMENITIES PROVIDED TO RESIDENTS. A GRANT WAS MADE BY AMAZON TO PROVIDE 50 ALEXA DOTS TO THE ASSISTED LIVING RESIDENTS. THE DOTS OPERATE ON AN ENTERPRISE LEVEL CAMPUS WI-FI SYSTEM. RESIDENTS ARE NOT REQUIRED TO HAVE A SMART PHONE OR TO HAVE AN ACCOUNT WITH AMAZON. NO PRIVATE INFORMATION IS KEPT ON THE SYSTEM. WITHOUT TOUCHING ANY SCREEN, RESIDENTS CAN ASK ALEXA WHAT IS FOR LUNCH, WHAT IS ON THE MENU, CALL THE FRONT DESK, SECURITY, FITNESS CENTER, ACTIVITIES, BEAUTY SHOP, ROOM SERVICE, AND FRIENDS. THE ALEXAS PROVIDE RESIDENTS THEIR FAVORITE MUSIC, ADJUST THE VOLUME, READ THE BIBLE, AND OTHER BOOKS AVAILABLE. IT PLAYS TED TALKS, IT COACHES CHAIR AND BRAIN WORKOUTS, IT READS THE NEWS, CONTROLS THE THERMOSTAT, CHANGES THE TV CHANNEL, IT FINDS THEIR FAVORITE TV PROGRAMS, AND THE CHANNEL AND TIME TO WATCH, IT CONTROLS THE APARTMENT LIGHTS, NUKES THE FOOD IN THE MICROWAVE, IT PROVIDES DIRECTIONS TO THE LOCATIONS OF THE SWIMMING POOL, WORKOUT CENTER, LIBRARY, GATHERING PLACE, TO-GO WINDOW, CONVENIENCE STORE, THE DATE, TIME, AND LOCATION OF CAMPUS ACTIVITIES. IT AUTOMATICALLY PREPARES WORK ORDERS AND EMAILS THEM TO THE FRONT DESK. ALEXA REMINDS RESIDENTS WHEN IT IS TIME TO TAKE THEIR MEDICATIONS OR GO TO AN EVENT.TO HELP CONSERVE ELECTRICITY AND PREVENT FALLS, FELLOWSHIP SQUARE-MESA APPLIED FOR AND RECEIVED A GRANT FROM SALT RIVER PROJECT, THE LOCAL ELECTRIC UTILITY PROVIDER. THE GRANT PROVIDED WI-FI CONTROLLED THERMOSTATS IN EVERY RESIDENT'S APARTMENT AND IN ALL COMMON AREAS. ALL RESIDENTS CAN NOW USE THEIR VOICE TO CONTROL THEIR INDIVIDUAL THERMOSTAT WITHOUT HAVING TO GET UP TO ADJUST IT. STAFF CAN NOW CONTROL FUNCTIONS AND HELP SET INDIVIDUAL THERMOSTATS TO ADJUST TEMPERATURES REMOTELY USING SOFTWARE THAT TRACKS UTILITY USAGE AND MONITORS THE EQUIPMENT FOR MALFUNCTIONS. WHEN RESIDENTS LEAVE COMMON AREAS OR THEIR APARTMENTS THE TEMPERATURE AUTOMATICALLY SETS TO "ECONOMY MODE".FOUNDED IN 1979, THE CHRISTIAN CARE COMPANIES BEGAN WITH 96 UNITS OF SUBSIDIZED HOUSING AND A 60-BED NURSING CENTER ON THE PHOENIX CAMPUS. THE ORGANIZATION IS ONE OF 24 RELATED TAX-EXEMPT CHRISTIAN CARE COMPANIES (THE NAMES OF THE OTHER 23 ARE LISTED ON SCHEDULE R) WHO PROVIDE HOUSING, HOSPITALITY, AND HEALTHCARE SERVICES FOR VERY-LOW-TO-MIDDLE INCOME ARIZONA SENIORS. THE CHRISTIAN CARE COMPANIES WERE FOUNDED BY THE NONDENOMINATIONAL CHRISTIAN CHURCHES OF ARIZONA AND ARE GOVERNED BY CORPORATE BOARDS OF DIRECTORS. AT DECEMBER 31, 2021, THE CHRISTIAN CARE COMPANIES OWN AND OPERATE 1,990 UNITS OF MIDDLE-INCOME, MARKET-RATE INDEPENDENT, ASSISTED, AND MEMORY CARE UNITS FOR SENIORS ON SIX CAMPUSES, 338 UNITS OF SUBSIDIZED HOUSING, AND A 68-BED HEALTH CENTER. IN ADDITION, WE PROVIDE ASSISTED LIVING SERVICES TO UP TO 106 RESIDENTS OF OUR SUBSIDIZED HOUSING.BENEFITS OF BEING PART OF A CHRISTIAN CARE COMMUNITY TYPICALLY INCLUDE SHARED HOUSEKEEPING; MAINTENANCE AND SECURITY SERVICES, INCLUDING CALL BUTTON MONITORING; ACCESS TO PRIVATE SCHEDULED TRANSPORTATION; AND COMPUTER LABS WITH HIGH-SPEED INTERNET. RESIDENTS TYPICALLY HAVE ENTREE TO ON-CAMPUS FOOD SERVICE VENUES, ENTERTAINING ACTIVITY PROGRAMS, HEALTH & WELLNESS OPPORTUNITIES AND CHAPLAINCY SERVICES.IN 2020 FELLOWSHIP SQUARE-MESA WAS NOMINATED AND WAS AWARDED THE CHAMPION OF ENERGY EFFICIENCY BY SALT RIVER PROJECT. THE WORK DONE EARLIER BY FELLOWSHIP SQUARE-MESA WITH SMART THERMOSTATS DISPLAY HOW CREATIVE THINKING AND THOUGHTFUL INTEGRATION CAN IMPROVE THE DAILY LIVES OF COMMUNITY RESIDENTS. THE COMMUNITY WILL BENEFIT BY AN ESTIMATED $32,104 ANNUAL ENERGY COST REDUCTION BECAUSE OF THE 413,708 KILOWATT-HOUR (KWH) SAVINGS ACHIEVED THROUGH THE UPGRADE. THE COMMON AREAS WERE UPGRADED WITH LED LIGHTING. THE ESTIMATED ANNUAL ENERGY COST SAVINGS IMPACT FOR THE LIGHTING PROJECT IS $15,620 WHICH IS THE RESULT OF A 192,966-KWH YEARLY REDUCTION.
