THE COMMUNITY HEALTH WORKER PROGRAM IS A PARTNERSHIP WITH THE CITY OF NEW ORLEANS INTENDED TO DEPLOY A COMMUNITY HEALTH OUTREACH PROGRAM MODEL IN TARGETED NEIGHBORHOODS THROUGHOUT THE CITY. THIS PROGRAM ACTS AS A SUPPLEMENT TO CONTACT TRACING AND TESTING EFFORTS, ENSURING THAT THOSE COMMUNITIES HARDEST HIT AND HARDEST TO REACH WILL GET APPROPRIATE ATTENTION AS WELL AS REFERRALS TO NEEDED HEALTH ECONOMIC, AND SOCIAL SERVICES.
