THE ORGANIZATION EMPLOYED A MULTIDISCIPLINARY TEAM MODEL TO PROVIDE DIRECT GUARDIANSHIP SERVICES TO A POPULATION OF OLDER ADULTS AND PEOPLE LIVING WITH DISABILITIES AND MENTAL ILLNESS WHO NEED HELP MAKING DECISIONS, HAVE NO FAMILY OR FRIENDS TO FILL THE ROLE, AND LARGELY HAVE VERY LOW INCOMES.
