("HOSPICE") COORDINATED PROGRAM OF HOME AND IN-PATIENT CARE, WHICH PROVIDES PHYSICAL, EMOTIONAL AND SPIRITUAL CARE FOR TERMINALLY ILL PATIENTS AND THEIR FAMILIES AND SUPPORTS THE CHARITABLE PURPOSES OF HOSPICE. PALLIATIVE AND SUPPORTIVE CARE PROVIDED BY THE HVMHC IS APPROPRIATE FOR PATIENTS WITH SERIOUS OR LIFE LIMITING ILLNESSES AND IS FOCUSED ON PROVIDING RELIEF FROM THE SYMPTOMS AND STRESS OF THE ILLNESS WITH THE GOAL OF IMPROVING QUALITY OF LIFE FOR BOTH THE PATIENT AND THE FAMILY.
