THE NETWORK WORKS TO IMPROVE POPULATION HEALTH (THE STATE OF COMPLETE PHYSICAL, MENTAL, AND SOCIAL WELL-BEING) IN KITTITAS COUNTY THROUGH CROSS-SECTOR COLLABORATION AND SYSTEMS CHANGE. THIS COLLABORATION IS GUIDED BY A COMMUNITY HEALTH IMPROVEMENT PLAN COMPLETED IN COORDINATION WITH THE HEALTH DEPARTMENT.
