NJDHS RHH PROGRAM TO CREATE A THRIVING AND SUSTAINABLE COMMUNITY COALITION DEDICATED TO SIGNIFICANTLY IMPROVING THE HEALTH AND OVERALL QUALITY OF LIFE FOR RESIDENTS OF THE GREATER PASSAIC COUNTY AREA BY SPECIFICALLY ADDRESSING SOCIAL DETERMINANTS OF HEALTH. ST. JOSEPH'S DONATED APPROXIMATELY $240,000 OF EXPENSES FOR THIS PROGRAM.
HCPC WORKED WITH KEY LOCAL CLINICAL, COMMUNITY, AND FAITH-BASED PARTNERS TO ENHANCE THE LOCAL PUBLIC HEALTH INFRASTRUCTURE, PROVIDE COVID-19 EDUCATION AND OUTREACH, AND FACILITATE LOCAL COVID-19 VACCINATIONS. MORE SPECIFICALLY, HCPC EXECUTED FORMAL AGREEMENTS WITH CLINICAL AND NON-CLINICAL PARTNERS WHO SERVE RESIDENTS OF PATERSON AND PASSAIC. THIS FUNDING ENABLED SUCCESS IN ACHIEVING PROGRAM GOALS OF: 1. ADDRESS VACCINATION HESITANCY THROUGH PARTNERS WITH DIRECT COMMUNITY REACH AND PROVIDE CLEAR CONSISTENT MESSAGING2. FACILITATE DIRECT APPOINTMENT AND LOCAL ACCESS TO STABLE COVID-19 VACCINATION3. BUILD LOCAL PUBLIC HEALTH CAPACITY WITH PARTNER CBO AND FBOS THROUGH THE DEVELOPMENT OF NON-CLINICAL PUBLIC HEALTH PARTNERS PROGRAM.ST. JOSEPH'S DONATED APPROXIMATELY $48,000 OF EXPENSES FOR THIS PROGRAM.
FAITH IN PREVENTION UNIFY COMMUNITIES, IMPROVE HEALTH OUTCOMES FOR GREATER PASSAIC COUNTY AREA AND ASSIST NEW JERSEY IN ADVANCING IMPORTANT STATE HEALTH PRIORITIES.. RECRUIT FAITH COMMUNITIES: HCPC WILL LEVERAGE ITS PARTNERSHIPS WITH A BROAD CROSS-SECTION OF ORGANIZATIONS TO IDENTIFY POTENTIAL FAITH COMMUNITIES FOR PROGRAM IMPLEMENTATION. THIS STRATEGY INCLUDES ESTABLISHING RELATIONSHIPS AND SHARING INFORMATION ABOUT FAITH IN PREVENTION (FIP) WITH HCPC COMMUNITY ADVISORY BOARD (CAB) MEMBER ORGANIZATIONS, LOCAL FAITH-BASED (FBOS) AND COMMUNITY BASED ORGANIZATIONS (CBO) IN PATERSON IN ORDER TO ASSIST WITH RECRUITMENT EFFORTS.
