TO IMPROVE ACCESS TO HEALTHCARE, HYGIENE CARE, SOCIAL SERVICES, AND TO PROVIDE MEDICAL RESPITE HOUSING TO HAWAII'S HOMELESS AND UNDERSERVED POPULATIONS. TO PROVIDE WALK-IN MEDICAL CLINIC SERVICES IN HONOLULU.

OUR MEDICAL CLINIC IS LOCATED AT THE KANEOHE JOINT OUTREACH CENTER. WE OFFER WALK-IN MEDICAL AND SOCIAL SERVICES TO GENERALLY HOMELESS POPULATIONS IN HONOLULU. WE PARTNER WITH THE INSTITUTE FOR HUMAN SERVICES AND OTHER SOCIAL SERVICES ORGANIZATIONS TO PROVIDE SERVICES TO OUR HOMELESS PATIENTS. H4'S MEDICAL RESPITE SERVICE, WHICH OPENED IN FEBRUARY 2021 PROVIDES 24-HOUR RESIDENCE WITH MEAL SERVICE FOR HOMELESS INDIVIDUALS WHO ARE TOO ILL OR FRAIL TO RECOVER FROM AN ACUTE ILLNESS OR INJURY ON THE STREETS BUT ARE NOT SICK ENOUGH TO MEET INPATIENT LEVEL OF CARE CRITERIA FOR A HOSPITAL OR NURSING FACILITY. MEDICAL RESPITE PROVIDES A CLEAN AND SAFE ENVIRONMENT FOR A FEW DAYS TO A FEW WEEKS, WITH MEDICAL AND SOCIAL SERVICE SUPPORT, FOR PATIENTS IN RECOVERY OR RECEIVING NEAR-DEATH HOSPICE CARE. AS AN FYI, H4 OPERATED OUR KANEOHE JOINT OUTREACH CENTER UNTIL JUNE 30TH, 2022.
