SCOTT & WHITE CARE PLANS, DBA BAYLOR SCOTT & WHITE CARE PLAN (SWCP), WAS CREATED IN 2016 AS A TEXAS NONPROFIT HMO. SCOTT AND WHITE HEALTH PLAN (SWHP) IS THE SOLE MEMBER OF SWCP. SWCP WAS FORMED WITH A SOCIAL WELFARE PURPOSE WITH THE INTENT TO OFFER MEDICARE, MEDICARE ADVANTAGE, MEDICAID AND OTHER HMO PLANS. TOGETHER WITH SWHP, SWCP'S SERVICE AREA ENCOMPASSES 171 COUNTIES AND SWCP PROVIDES COVERAGE TO OVER 30,000 MEMBERS IN NORTH, CENTRAL AND WEST TEXAS. SWCP IS PART OF BAYLOR SCOTT & WHITE HEALTH (BSWH), A TEXAS NONPROFIT, FAITH-BASED INTEGRATED HEALTH CARE DELIVERY SYSTEM THAT PROVIDES PATIENT CARE, MEDICAL EDUCATION, MEDICAL RESEARCH AND OTHER COMMUNITY SERVICES TO THE RESIDENTS OF NORTH AND CENTRAL TEXAS. SWCP, WITH ITS PARENT SWHP, HAS DEMONSTRATED A COMMITMENT TO THE HEALTH AND WELL-BEING OF NOT ONLY ITS MEMBERS BUT THE COMMUNITY AS A WHOLE THROUGH A NUMBER OF DIRECTED PROGRAMS OF VARYING PURPOSE SOME OF THESE PROGRAMS ARE DESIGNED TO ENGAGE MEMBERS IN THEIR OWN CARE OR ENCOURAGE A HEALTHY LIFESTYLE. OTHER PROGRAMS OFFER SUPPORT AND HELP COORDINATE THE DELIVERY OF CARE AT THE RIGHT PLACE, THE RIGHT TIME, AND FROM THE RIGHT PROVIDER, ESPECIALLY FOR THOSE MEMBERS WITH COMPLEX OR CHRONIC CARE ISSUES.
