FOR THE DURATION OF THE CURRENT COLLECTIVE BARGAINING AGREEMENTS BETWEEN THE INTERNATIONAL UNION, UNITED AUTOMOBILE, AEROSPACE, AND AGRICULTURAL IMPLEMENT WORKERS OF AMERICA (THE "UAW") AND FCA US LLC ("FCA"), FORD MOTOR COMPANY ("FORD"), AND GENERAL MOTORS LLC ("GM"), THE UAW-FCA-FORD-GENERAL MOTORS LEGAL SERVICES PLAN (THE "PLAN") PROVIDES LEGAL SERVICE BENEFITS, WHICH ARE WELFARE BENEFITS UNDER THE EMPLOYEE RETIREMENT INCOME SECURITY ACT OF 1974, AS AMENDED ("ERISA"), TO ELIGIBLE PLAN PARTICIPANTS. THE PERSONAL LEGAL SERVICES THAT ARE MADE AVAILABLE TO MEMBERS OF THE UAW ARE: (I) "OFFICE LEGAL SERVICES" FOR CERTAIN LEGAL MATTERS, (II) A LOW-COST REFERRAL TO A COOPERATING ATTORNEY AT A REDUCED RATE, AND (III) SOCIAL SECURITY DISABILITY SERVICES FOR EMPLOYEES AND RETIREES.

THE PLAN IS AN EMPLOYEE WELFARE BENEFIT PLAN THAT WAS ESTABLISHED PURSUANT TO AN AGREEMENT BETWEEN FCA, FORD, GM, AND THE UAW. THE PLAN PROVIDES LEGAL SERVICES AS A BENEFIT UNDER THE CURRENT NATIONAL CONTRACT BETWEEN THE UAW AND FCA, FORD, AND GM. THE PLAN IS SUBJECT TO THE PROVISIONS OF ERISA.
