THE BRIDGE PROGRAM IS A TRANSITIONAL SUPPORT MODEL SERVING YOUNG PEOPLE AGES 14 TO 29, TO ASSIST THOSE YOUTH WHO ARE AGING OUT OF FOSTER CARE AND LACK THE NECESSARY SKILLS, RESOURCES, AND SUPPORT TO CONTINUE DEVELOPMENT IN ADULTHOOD. UTILIZING A STRENGTHS-BASED AND PERSON-CENTERED ENVIRONMENT AND CONNECTED TO A NETWORK OF SUPPORT. EACH YOUNG PERSON ACCEPTED INTO THE BRIDGE IS ASSIGNED A MULTISYSTEMATIC NAVIGATOR TO ASSIST WITH THEIR TRANSITION AND COORDINATE WITH THE DEPARTMENT OF HEALTH AND HUMAN SERVICES (DHHS), AND OTHER SERVICE AGENCIES PROVIDING ASSISTANCE. THE NAVIFATOR PROVIDES COACHING AND SUPPORT ACROSS FIVE TRANSITIONAL LIFE DOMAINS: EDUCATION, EMPLOYMENT, LIVING SITUATION, COMMUNITY LIFE FUNCTIONING AND PERSONAL EFFECTIVENESS AND WELLBEING.
