THE LEAGUE'S FUNDRASING ACTIVITIES SUPPORT THE HEALTH & WELLNESS PROGRAM AT THE SANTA CLARA SENIOR CENTER. THE LEAGUE'S CONTRIBUTION SUPPLEMENTS OTHER SOURCES OF FUNDING FOR THE WELLNESS PROGRAM. THE HEALTH AND WELLNESS PROGRAM FACILITATES THE FOLLOWING SERVICES FOR CLIENTS OF THE SANTA CLARA SENIOR CENTER: HEALTH EDUCATION AND COACHING, SOCIAL ENGAGEMENT AND SUPPORT, EXPLANATION OF AND REFERRAL TO OTHER SERVICE PROVIDERS, HEALTH PROMOTION ACTIVITIES, VISITS TO HOMEBOUND SENIORS.
