THE HEALTH AND WELLNESS PROGRAM FACILITATES THE FOLLOWING SERVICES FOR CLIENTS OF THE SANTA CLARA SENIOR CENTER: HEALTH EDUCATION AND COACHING, SOCIAL ENGAGEMENT AND SUPPORT, EXPLANATION OF AND REFERRAL TO OTHER SERVICE PROVIDERS, HEALTH PROMOTION ACTIVITIES, VISITS TO HOMEBOUND SENIORS.
