GRAYSON ST SAAF PROVIDED CLIENT SERVICES FOR 2,683 PEOPLE LIVING WITH HIV (PLWH), CONNECTING CLIENTS TO NEEDED RESOURCES INCLUDING HOUSING, PRESCRIPTION ASSISTANCE, MEDICAL APPOINTMENTS, APPLICATIONS FOR FINANCIAL ASSISTANCE AND MORE.MEDICAL CASE MANAGEMENT WAS PROVIDED TO 414 INDIVIDUALS AND NONMEDICAL CASE MANAGEMENT TO 521 CLIENTS; 69 PLWH RECEIVED RENTAL ASSISTANCE FOR A TOTAL OF 1,735 MONTHS. EMERGENCY FINANCIAL ASSISTANCE WAS PROVIDED TO 111 PLWH AND AIDS PHARMACEUTICAL ASSISTANCE WAS PROVIDED TO 35 INDIVIDUALS. TRANSITIONAL HOUSING WAS PROVIDED TO 40 CLIENTS FOR 3,201 DAYS OF HOUSING.
