THE CARE COORDINATION PROGRAM USES A CLIENT-CENTERED, COLLABORATIVE APPROACH TO ADDRESS CLIENT CARE, WHICH HELPS ESTABLISH THE FOUNDATION FOR A HEALTHY AND PRODUCTIVE PARTNERSHIP. CARE COORDINATORS SUPPORT THE ACHIEVEMENT OF CLIENT GOALS BY PROVIDING TAILORED CARE AND EXPERTISE THROUGHOUT ALL PHASES OF LIVING WITH HIV/AIDS. SERVICES INCLUDE HEALTHCARE NAVIGATION AND ENROLLMENT, MEDICATION AND MEDICAL LAB CO-PAY ASSISTANCE, AND TRANSPORTATION ASSISTANCE.
THE TULSA CARES HOUSING PROGRAM ADDRESSES UNSTABLE HOUSING AND SERVES AS A GATEWAY INTO CARE, THUS IMPROVING THE EFFECTIVENESS OF OTHER PROGRAMS AND EMPOWERING CLIENTS TO BETTER FOCUS ON THEIR HEALTH. TULSA CARES OFFERS FINANCIAL ASSISTANCE TO QUALIFIED CLIENTS AND THEIR FAMILIES, INCLUDING LONG-TERM PLANNING, MORTGAGE, RENT, UTILITY, AND DEPOSIT ASSISTANCE.
