Describe the Organization's Program Activity:
THE ORGANIZATION PROVIDES MEDICAL SERVICES TO PATIENTS. IN 2021, LA FE'S COMMUNITY HEALTH CENTERS HAD 62,489 MEDICAL ENCOUNTERS. IMPROVEMENTS TO ACCESS TO PRIMARY CARE WERE ACCOMPLISHED THIS PROJECT PERIOD THROUGH STRATEGIC SERVICES AND EXPANSIONS. THIS PROCESS ALLOWED LA FE TO CONVERT ITSELF INTO A FRONT-LINE COMMUNITY ASSET AND MEDICAL HOME FOR THE MANY FAMILIES THAT HAVE BECOME VICTIMS OF OUR CHALLENGING NATIONAL ECONOMY. IN 2021, LA FE'S COMMUNITY HEALTH CENTERS HAD 6,033 DENTAL ENCOUNTERS THROUGH ITS OVER 50 YEARS OF UNINTERRUPTED COMMUNITY SERVICE, LA FE HAS BECOME A STABLE AND RELIABLE LEADER IN THE ESTABLISHMENT OF A HEALTH CARE SAFETY NET TO CONTINUE TO ADDRESS OUR COMMUNITY'S HEALTH DISPARITIES.
