CLIENTS ARE PROVIDED WITH BASIC NEEDS, INCLUDING MEALS, AND ADDITIONAL SERVICES TO HELP THEM MANAGE HEALTH RELATED CONDITIONS. ADDITIONALLY, CASE MANAGEMENT WORKS WITH CLIENTS TO IDENTIFY AND ADDRESS NONMEDICAL NEEDS, CONNECT THEM TO BENEFITS AND RESOURCES, AND TO OVERCOME THE BARRIERS TO HOUSING.
