SERVICES OFFERED TO PATIENTS INCLUDE PRIMARY CARE, BEHAVIORAL HEALTH, GENERAL DENTISTRY, NUTRITION, OUTREACH AND ENROLLMENT, AND COORDINATED CARE THROUGH A TEAM OF QUALITY SUPPORT SPECIALISTS.
