EVERGREEN PROVIDES COMMUNITY SETTINGS FOR PEOPLE WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES WITH AN EMPHASIS ON ACTIVE TREATMENT DESIGNED TO HELP THE INDIVIDUAL BECOME MORE INDEPENDENT. IN ADDITION, SUPPORT IS ALSO PROVIDED FOR THOSE WHO LIVE IN THEIR OWN APARTMENT OR HOME ACCORDING TO THE NEEDS OF EACH INDIVIDUAL. EVERGREEN IS LICENSED AND CERTIFIED AS A TITLE XIX MEDICAID PROVIDER.
