SENIOR TRANSPORTATION PROGRAM WHICH ENABLES OLDER ADULTS TO MAINTAIN THEIR INDEPENDENCE AS LONG AS POSSIBLE BY PROVIDING ACCESS TO AVAILABLE COMMUNITY SERVICES INCLUDING MEDICAL FACILITIES, PHARMACIES, GROCERY STORES, AND OTHER ESSENTIAL SERVICES. APPROXIMATELY 5,069 TRIPS(UNITS OF SERVICE) WERE PROVIDED TO INDIVIDUALS.


HOME DELIVERED MEALS PROGRAM WHICH PROVIDES NUTRITIONALLY BALANCED MEALS TO HOMEBOUND INDIVIDUALS. APPROXIMATELY 150,387 MEALS WERE DELIVERED TO ELIGIBLE INDIVIDUALS.


HOMEMAKER SUPPORTIVE SERVICES PROGRAM WHICH PROVIDES IN-HOME DAILY LIVING SERVICES TO ALLOW IMPAIRED AND/OR FRAIL ELDERLY INDIVIDUALS TO REMAIN IN THEIR HOMES, APPROXIMATELY 4,450 HOURS (UNITS OF SERVICE) WERE PROVIDED TO QUALIFYING INDIVIDUALS.
