SOJOURNS PROVIDERS SPEND MORE TIME WITH PATIENTS AND COLLABORATE WITH EACH OTHER TO DELIVER THE MOST COMPREHENSIVE AND INTER-DISCIPLINARY CARE. AS A NONPROFIT CLINIC SERVING THOSE WHO NEED US THE MOST, OUR MODEL REFLECTS AN INTENTION TO ELEVATE AND HUMANIZE HEALTHCARE IN A SYSTEM THAT HAS BEEN FRAGMENTED, OVER-SYSTEMATIZED AND MONETIZED.
