("CHFC") OPENED ON JANUARY 2, 1996 TO PROVIDE BASIC MEDICAL AND DENTAL DIAGNOSTIC AND TREATMENT SERVICES FOR THE UNINSURED AND UNDERINSURED OF SOUTH-CENTRAL KENTUCKY. CHFC ALSO SERVES INDIVIDUALS WHO MAY BE COVERED BY SOME FORM OF INSURANCE, BUT WHO ARE UNABLE TO ACCESS A PROVIDER FOR ACUTE OR CHRONIC HEALTH ISSUES. SEE SCHEDULE O FOR ADDITIONAL INFORMATION.
