SUNCOAST HOSPICE FOUNDATION SEEKS TO FOSTER RELATIONSHIP BUILDING OF COMMUNITY INVESTMENTS IN ORDER TO SUPPORT UNFUNDED SERVICES, FUND START-UP SERVICES AND TO PROVIDE RESOURCES TO EMPATH HEALTH, INC.'S AND ITS MEMBERS' PROGRAMS. EMPATH HEALTH, INC. IS A NONPROFIT, INTEGRATED NETWORK OF CARE ENCOMPASSING HOSPICE, SKILLED HOME HEALTH CARE, ASSISTANCE FOR INDEPENDENT LIVING AND DAYCARE FOR SENIORS, COMMUNITY-BASED PALLIATIVE CARE, SERVICES FOR THOSE INFECTED OR AFFECTED BY HIV, ADVANCE CARE PLANNING AND GRIEF SERVICES.PROGRAMS RECEIVING SUPPORT THROUGH SUNCAOST HOSPICE FOUNDATION INCLUDE PROGRAMS OF SUNCOAST HOSPICE, SUNCOAST PACE, EMPATH PARTNERS IN CARE (EPIC), EMPATH HOME HEALTH CARE, INC., EMPATH COMMUNITY HEALTH AND OTHER PROGRAMS AND SERVICES OF EMPATH HEALTH, INC.
